TROOP  39  PERMISSION  SLIP

EVENT: Summer Camp at Camp Long Lake

 

COST: $190.00 by Feb. 14, 2008 or for any New Scout; $210.00 by May 15, 2008; $220.00 by July 3, 2008; or $250.00 at the door.

 

RETURN SLIP BY: Feb. 14, 2008 for $190.00 Fee; May 15, 2008 for $210.00 Fee; July 3, 2008 for the $220.00 Fee (For New Scouts Only -- $190.00 fee is due by July 3, 2008).

 

This Troop 39 outing will be at Camp Long Lake from Sun. July 6, 2008  to Sat. July 12, 2008.  Scouts will leave from the Troop House at approximately 11:00 AM on Sunday and will be returned to their home about 11:00 AM on Saturday. 

 

What to Bring:  Usual Camping Gear. (A Detailed List of other items will be provided at a later date).

** You must have a current Class II Health Form turned in by May 15, 2008!!

The Scout's health insurance provides primary coverage.  Troop 39 provides secondary health insurance coverage and BSA provides liability coverage for adult leaders.

I, the parent or guardian of __________________________________________, hereby give my permission for him to participate in this Troop 39 activity at Camp Long Lake from Sun. July 6, 2008  to Sat. July 12, 2008.

 

In case of an emergency, I understand every effort will be made to contact me.  If I can’t be reached, I give my permission to the adult leaders to secure proper medical treatment for the above named scout.  I have previously provided the insurance policy information and emergency phone numbers.

 

I understand that if my son behaves in an unacceptable manner, the adult leader in charge will warn him that he will be sent home if such behavior continues and will discuss the situation with me.  If his unacceptable behavior continues after this warning, I will be asked to pick him up.
